 Description:  Locations of water monitoring wells.

      Source:  Kentucky Department for Natural Resources (DNR) 
               Surface Mining Information System (SMIS) database.  

      Origin:  Latitude and longitude data are submitted by the 
               permittee with the permit application.  In some cases,
               latitude and longitude have been field-verified by 
               DSMRE personnel. The SMIS database is used for tracking
               all permitting and enforcement actions for coal mining
               activity in the Commonwealth of Kentucky including the
               current status of all pending and completed permitting
               activity. DNR continuously updates and maintains the
               SMIS database, from which the WATER layer is updated 
               periodically.
 
  Attributes:  Information describing each water monitoring well is 
               contained in the dBASE file, WATER.DBF. Each record 
               contains the following items:

                  PERMIT_NUM    - permit number
                  PER_NAME      - permittee
                  LATITUDE_D    - latitude (decimal degrees)
                  LONGITUDE_    - longitude (decimal degrees)
                  QUAD_DESC     - quadrangle name
                  MINE_STATU    - mine status*
                  FACILITY_I    - well id
                  WATER_MONI    - water monitoring type**
