Division of Mine Permits electronic forms are Microsoft Word templates that have been compressed into Zip files and are available for downloading. To access the files you will need a compression utility such as WinZip which can be downloaded here.  Once the forms have been unzipped, they may be opened in Microsoft Word.  The user can then complete the forms including additional sections and attachments where appropriate.

Instructions for Downloading and Opening Electronic Forms:

• Click the link for the desired Division of Mine Permits form.
• In the File Download window select the Save option.
• In the Save As window navigate to the folder where the file is to be saved and type in a filename or accept the default filename.
• Using My Computer or Windows Explorer, navigate to the location where you saved the file.  It will appear with the filename and the WinZip icon.
• Double-click the icon to open the compressed file. 
• In the WinZip window, click the Extract button or choose Actions > Extract from the WinZip menu.
• In the Extract window, browse to the desired folder on your computer where you want to store the forms or accept the default location.
• Click the Extract button. Close the WinZip window.
• Start Microsoft Word and open the file from the location you specified.  You should select the Enable Macros option (middle tab) when this option is presented in a box in the center of the screen.

The Attach Tools Microsoft Word template allows you to add attachments to your forms.  Download Attach Tools in the same way you download an electronic form.
